Collin County voters who want to cast a mail ballot in the Nov. 3 election have until 5 p.m. Friday, Oct. 23, to get their application to the county's early voting clerk. That is a received-by deadline, not a postmark date, according to the county's election notice and the Texas Secretary of State's guidance as summarized by NBC 5.
Getting an application
The official application form is available online, through the Secretary of State or from the county early voting clerk, NBC 5 reported.
A voter may instead write out a request, provided it lists the voter's name, the address where the voter is registered, the address where the ballot should be mailed if different and allowed by law, a valid reason for voting by mail, the election being requested and the voter's or a witness's signature.
Every application must carry an identification number: a Texas driver's license number, a Texas personal ID number or an election identification certificate number from the Department of Public Safety. The voter registration number does not count.
Voters who have none of those can use the last four digits of their Social Security number, and anyone with none of the numbers must check the box saying so.
Voters 65 or older or with a disability can mark "Annual Application" to request mail ballots for every election in the calendar year with one form. Voters citing disability must also include a signed affirmation, worded in state guidance, that an illness or physical condition would make voting in person risky for their health or likely to require assistance.
